CHLEBAK, Sophia At the Juravinski Hospital on Thursday, August 30, 2012, in her 77th year. Beloved wife of the late Ralph Chlebak. Predeceased by her daughter Caroline. Dear sister of Fran, Stella, Irene (Jack). Survived by brother-in-law Art (Rita), sister-in-law Rose and many nieces and nephews. Special thanks for the thoughts and prayers of her many friends and Dr. Pinthus and team and the nurses at the Juravinski Hospital for the their care. Friends will be received at the Donald V. Brown Funeral Home, 36 Lake Avenue Drive, Stoney Creek on Monday from 2 to 5 p.m. Vigil for the deceased at 4:30 p.m. Funeral Mass to be held at St. Francis Xavier Church, Stoney Creek on Tuesday, September 4, 2012 at 11 a.m. Interment Pine Hills Cemetery, Scarborough.
